We would like to inform you that we have decided to implement the university-based vaccination program for the novel coronavirus as follows. We recommend that as many people as possible take this opportunity to receive the vaccine.

Details

Vaccination period
・1st dose: Monday, July 26, 2021 - Sunday, August 1, 2021
・2nd dose: Monday, August 23, 2021 - Sunday, August 29, 2021
Vaccination hours and appointment slots
・10:00-12:00 (300 people)
・13:30-15:30 (300 people)

Communication Plaza (Reception: 2nd floor)
How to make an appointment
・Faculty members (including part-time lecturers) who wish to be vaccinated must make a reservation through the web-based reservation system.
・The web-based reservation system is being prepared so that reservations can begin on July 16 (Fri.) The address of the web-based reservation system and the reservation procedure will be announced in the third report by July 14 (Wed.) at the latest.
・The reservation system is based on a set of first and second appointments.(The second session will be held four weeks later on the same day of the week at the same time.)
・Since the period from July 19 to July 25 (August 16 to August 22) is the vaccination period for Nagoya University of Arts and Sciences, etc., faculty members of Nagoya University of Foreign Studies cannot make reservations.

Notes
・The vaccination is for all students, faculty and staff, but it is not compulsory. Failure to receive the vaccine will not result in any disadvantageous treatment within the university.
・Please do not criticize or discriminate against those who do not receive vaccinations.
・You may receive the vaccine at your local government office, but please do not make a double appointment.
・The vaccine is provided by Moderna. Before applying for vaccination, please make sure you have read and understood the information about the vaccine.

What to bring on the day
Please note that if you forget to bring the following items, you will not be able to receive the vaccination that day.
(1) ID card (employee ID card issued by the corporation employee ID card issued by the corporation or Residence card)
(2) Completed Japanese version of Prevaccination Screening Questionnaire for COVID-19 vaccine. (Only the Japanese version is accepted by Japanese doctors and local government office. If you fill in the English version, you can copy it exactly to the Japanese document and bring the Japanese version.)
(3) Japanese version of Record of Vaccination or Vaccination Ticket (Coupon or Voucher, issued by the city or ward office) if you have received it already.
(4) Health insurance card
* Please download the Japanese version of Prevaccination Screening Questionnaire for COVID-19 vaccine and the Japanese version of Record of Vaccination, follow the example in English, fill in the necessary information on the Japanese versions, and bring both papers with you.
* If you do not receive your vaccination ticket (Coupon or Voucher) before the first visit, please be sure to bring it with you on the second visit. I.E. You can be vaccinated without your Vaccination ticket (Coupon or Voucher) for the first visit if you bring a Record of Vaccination (you can download here) when you go in for the 2nd dose.
